Supported Independent Living
Global Care Network specialises in providing Supported Independent Living services. We can assist you with:

• Setting up your home with the appropriate equipment so that you may live independently
• Providing support 24/7 (or as required) within your home

Applying for Funding from the NDIS
We have a comprehensive understanding of NDIS review processes. Global Care Network can help you with the following:
• Assisting you to prepare for NDIS plan review meetings
• Assistance applying for your first plan with the NDIS
• Access to our customised GCN toolkit to optimise your funding
Social and Community Participation
Our friendly Disability Support Workers can assist you with engaging with the community and social events. We provide support for a whole variety of activities, including:
• Attending events

Support Coordination
We have highly skilled Support Coordinators who can connect you to the informal, mainstream and community support services that you require.
• Our support coordinators work with you to choose your support team
• We will assist you finding the right place to live that meets your unique requirements
• Our support coordinators can connect you to the services as required to help improve your general health outcomes.
Transport and Travel Assistance
Global Care Network is able to provide assistance to get you where you need to go. This can be anything from:
• Travelling to and from work
• Travelling to and from social and community activities
• Travelling to and from medical appointments
• Our Support team can drive you in their own vehicle
• Providing assistance to catch public transport
• Accompanying you to ride a bike
• Holiday planning and preparation

With reliable industry connections to Allied Health Professionals, General Practitioners and medical specialists, Global Care Network can assist you with improving and maintaining your mental and physical health.
